Online Problem Report

DLSU students and faculty members may use this form to request assistance with certain services provided by the Help Desk.

All requests for assistance will be forwarded to the Help Desk. Be sure to fill in all required fields.

Note: Online Problem Report is not applicable for services that should be availed of by filling up a Service Request Form since they require students/faculty members to affix their signature on the form and personally report to the Help Desk. To know which services are available through an SRF, go to the Service Request Form page.

To make a computer laboratory reservation, complete the Computer Laboratory Reservation Form.
