Service Request Form

The Information Technology Center’s Service Request Form (SRF) shall be filled-up for requesting services from the office, which include:

  • Data Processing
  • Printing of Report
  • Master File Updating
  • Master File Printing
  • Web Page Updating
  • Web Page Design
  • Program Access Request
  • Forms Design
  • Graphics Design
  • System Development/Revision

Note: The SRF cannot be submitted online and requires requesting party to submit it personally to the ITC or by internal snail mail.

Memorandum of Agreement for Dial-up Access

For part-time faculty members to avail of the University’s Dial-up service, they need to sign the Memorandum of Agreement, which must be accomplished with the approval of the Vice Chancellor for Academics (VCA).

Note: Full-time faculty members need not accomplish the MOA, they can go directly to Help desk office or send an email at helpdesk@dlsu.edu.ph for request.

Help Desk shall activate the Dial-up account upon receipt of their copy of the MOA, and send an e-mail to the faculty member concerned informing that the dial-up account applied for has already been activated.
